El a fost fiul Despotului Gheorghie şi al Despotinei Irina. El şi sora lui Mara au trăit ceva vreme la curtea Sultanului Murat al II-lea. Lui i-au scos ochii turcii, la ledrene, precum şi fratelui lui, Grgur. El a devenit Despot al Serbiei la anul 1458. Curînd după aceea el a trebuit să se refugieze în Albania, unde s-a căsătorit cu Anghelina, fiica lui Skanderbeg. Orb şi cu destin tragic, dar plin de credinţă în Dumnezeu, Ştefan a trecut la cele veşnice în Italia, la anul 1468. Sfintele lui moaşte odihnesc la Mînăstirea Kruşedol, care a fost ctitorită de fiul lui, Maxim.
