Ca monah, Sfîntul Teodosie s-a asezat nu departe de orasul Tîrnovo din Bulgaria, unde a întemeiat o asezare monahala, care mai tîrziu a luat numele lui. El s-a aratat mare aparator al Ortodoxiei la un sinod din Bulgaria tinut împotriva bogomililor, la anul 1360 d. Hr., în care i-a rusinat cu desavîrsire pe bogomili, cu graiurile sale întelepte. El s-a savîrsit cu pace la Constantinopol, în anul 1362. Ucenicul sau, Roman, a continuat pîna la moarte viata de nevointe a Batrînului sau, în sihastria de acesta întemeiata, numita „a Sfîntului Teodosie."
